Services and prices

Each function is staffed, documented and reported through Apex, so nothing falls between vendors.

  • Medical billing

Claims sent through clearinghouses such as Availity and Optum, payments posted, and every claim followed to resolution.

  • Certified medical coding

AAPC-certified coders turn your documentation into compliant CPT and HCPCS codes.

  • Denial management

Denials worked by CARC and RARC code, root causes fixed and appeals filed, so the same denial does not come back.

  • AR recovery

Aged AR worked by payer and aging bucket until each claim is paid, appealed or written off with a reason.

  • Prior authorization

Authorizations secured before the visit, so procedures are not denied for lack of approval.

  • Provider credentialing

Payer enrollment and recredentialing, so claims are not held or denied for credentialing gaps.

  • Revenue cycle optimization

Charge capture, coding and denial patterns reviewed to recover missed revenue and lower your cost to collect.

  • Reporting and analytics

Collections, days in AR, denial rate and claim status in Apex, updated continuously.

Each specialty has its own payer mix, coding rules and denial patterns, and each has its own page. Every specialty pays the same rate.

How working with Quilven works

  1. Free audit

Share your AR aging totals. Within 48 hours you get a written report: where revenue is leaking, what we would recover and what the work would cost. You keep it either way.

  2. Scope, rate and a BAA

If you go ahead, we agree the scope and your rate, and sign a HIPAA Business Associate Agreement before any patient data is shared.

  3. Set up inside your EHR

We work in the system you already use, so you do not change software. Most practices are live within a few weeks.

  4. Live reporting in Apex

How to check a billing partner you just met

We do not publish client results yet because we have not earned the right to. When we do, each will be named, dated and stated with its sample size. Until then, these are things you can check.

  • Month-to-month terms

No multi-year contract. We have to earn your business every month, or you leave.

  • HIPAA and a signed BAA

Before any protected health information is exchanged, we sign a Business Associate Agreement under 45 CFR 160.103.

  • Documented workflow

Denials, appeals and AR follow-up follow a fixed cadence you can inspect, not a black box.

  • Cited benchmarks

Industry figures are cited to their source: MGMA and HFMA for definitions and benchmarks, HBMA for billing standards.

Who does the work

Your revenue cycle is run by named people with real credentials, not an anonymous queue. Named leads and their certifications are introduced during onboarding, with photos and direct contacts.

Questions practices ask

We charge 3 to 5 percent of net collections, with a $500 monthly minimum, not a flat monthly fee. The rate is set by monthly volume and is the same for every specialty. For comparison, industry percentage billing typically ranges from 4% to 9% of net collections, with the most competitive quotes between 5% and 7% (A3 Medical Billing, 2026; Neolytix, 2026).

No. Engagement is month to month, with 30 days' written notice to leave, so we have to earn your business every month. Your data and reports are yours and are provided on exit.

Yes. Before any protected health information is exchanged, we sign a HIPAA Business Associate Agreement under 45 CFR 160.103. Our coders are AAPC-certified and our workflows are documented.

We work in your existing system, including Athenahealth, eClinicalWorks, Epic, Tebra and Kareo, AdvancedMD, and NextGen. You do not change software to work with us.

We review AR aging by payer and bucket, top denial reasons by CARC and RARC code, payer mix, and clean-claim patterns. Within 48 hours you receive a written report showing where revenue leaks and what we would recover. The audit is free and obligation-free.

It depends on scope, your EHR, and payer enrollment status. Most practices are live within a few weeks. The audit, completed first, often shortens this by showing exactly where to start.

You do. Quilven works in your systems on your behalf; your data stays yours, and Apex gives you live access to it at all times.

No. Quilven augments your billing manager, it does not replace them. We take the payer grind: eligibility calls, claim scrubbing, denial triage, appeals, and AR follow-up. Your biller keeps oversight, local knowledge, and the relationship with your front office, and moves from chasing denials to preventing them, with Apex as the shared source of truth.

In-house billing depends on your staff having time, and it leaves gaps when someone is out; Quilven's coverage is team-backed. Other percentage billing companies often ask for an annual contract; Quilven is month to month. Offshore billing tends to be transactional, with thin appeals, and EHR-bundled RCM is limited to the EHR's own workflows. Quilven works denials by code with appeals, and you see everything live in Apex.

Independent, physician-owned specialty practices, including orthopedics, cardiology, gastroenterology, dermatology, ophthalmology, OB/GYN, urology, neurology, ENT, behavioral health, physical medicine and rehab, general surgery, and podiatry.

Independent practices with 3 to 25+ providers, and solo practices are welcome too. We are not built for enterprise, hospital-owned, or FQHC organizations.
